Zuclopenthixol decanoate
Indications/Uses
Maintenance treatment of schizophrenia & other psychoses, especially w/ symptoms eg, hallucinations, delusions, thought disturbances, in combination w/ agitation, restlessness, hostility, & aggressiveness.
Dosage/Direction for Use
IM Adult 200-400 mg (1-2 mL) every 2nd-4th wk. Inj vol >2 mL should be distributed between 2 inj sites. Individually adjust dose according to patient's condition.
Contraindications
Hypersensitivity. Decreased level of consciousness for any reason (eg, intoxication w/ alcohol, barbiturates or opiates), circulatory collapse, coma.
Special Precautions
NMS has been reported in patients receiving antipsychotics. Patients w/ pre-existing organic brain syndrome, mental retardation & opiate & alcohol abuse are over-represented among fatal cases. Treatment: Discontinuation of antipsychotics, symptomatic & general supportive in-patient treatment. Should be used w/ caution in patients w/ organic brain syndrome, convulsion, diabetes & advanced liver disease. Zuclopenthixol may modify insulin & glucose responses calling for adjustment of the antidiabetic therapy in diabetic patients. Zuclopenthixol may cause QT prolongation. Persistently prolonged QT intervals may increase the risk of malignant arrhythmias. Should be used w/ caution in susceptible individuals & in patients w/ a history of CV disorders. Concomitant treatment w/ other antipsychotics should be avoided. Cases of VTE have been reported w/ antipsychotic drugs. Leukopenia, neutropenia & agranulocytosis have been reported w/ antipsychotics. Long-acting depot antipsychotics should be used w/ caution in combination w/ other medicines known to have a myelosuppressive potential. Older people: Cerebrovascular: Zuclopenthixol should be used w/ caution in patients w/ risk factors for stroke. Increased mortality in older people w/ dementia: Clopixol is not licensed for the treatment of dementia-related behavioural disturbances. Careful dosing &, if possible, a serum level determination is advisable in patients w/ reduced liver function. Should not be used during pregnancy unless clearly necessary. Should be used w/ caution during lactation. Not recommended in childn.
Adverse Reactions
Somnolence, akathisia, hyperkinesia, hypokinesia, dry mouth. Increased appetite, increased wt, insomnia, depression, anxiety, nervousness, abnormal dreams, agitation, decreased libido, tremor, dystonia, hypertonia, dizziness, headache, paraesthesia, impaired conc, amnesia, abnormal gait, visual accommodation abnormalities, visual disturbance, vertigo, tachycardia, palpitation, hypotension, orthostatic hypotension, blocked nose, increased salivation, constipation, vomiting, dyspepsia, diarrhoea, excessive sweating, pruritus, myalgia, urinary disturbances, urinary retention, polyuria, asthenia, fatigue, discomfort, pain.
Drug Interactions
Zuclopenthixol may potentiate the sedative effect of alcohol & the effects of barbiturates & other CNS depressants. Neuroleptics may increase or reduce the effect of antihypertensive drugs. Concomitant use of neuroleptics & lithium increases the risk of neurotoxicity. TCAs & neuroleptics mutually inhibit the metabolism of each other. Zuclopenthixol may reduce the effect of levodopa & the effect of adrenergic drugs. Concomitant use of metoclopramide & piperazine increases the risk of extrapyramidal disorder. Concomitant use of drugs known to inhibit enzyme CYP2D6 may lead to decreased clearance of zuclopenthixol. Co-administration of drugs that prolong the QT interval is not recommended.
